ALL REQUESTS MUST BE MADE IN WRITING

• Full Name (include maiden name if married)

• Address

• Phone number

• Email address

• Date of birth

• Graduation year

• $3.00 for each transcript (check payable to PCHS)

• Name and address of WHERE the transcript should be mailed
