Monthly Newsletters
Every month we send a newsletter to the parents of our seniors. The newsletter gives important information that will help make senior year easier. You can find information on financial aid, upcoming college representative visits, scholarships, and much more! If you’ve misplaced your copy, you can easily look up the information in our archived newsletters to the right.

Documents & Forms for Seniors
The college counselor meets with the entire senior class at the beginning of the school year. The following are handouts provided for the seniors.

Application Procedures
This handout gives detailed information on the application process at Providence. These rules are to be followed by all students.

Application Tracking Slip
This form MUST accompany ALL transcript requests whether you apply online or with a paper application.

College Planning and Letter of Recommendation Packet
We ask that the students give us information about themselves. Counselors use this packet to help with writing personal letters of recommendation. The information is also used for scholarship purposes.

Info Sheet for Teacher Letter of Rec
This form is filled out by the student for each teacher/coach who will be writing a letter of recommendation. After completed, this form is returned to Mrs. Campbell for review. After her review, it is returned to the student who will then give it to the teacher/coach who is writing the letter of recommendation.

Parent Brag Sheet
This gives the parents a chance to brag about their student. We will use this information to help us with writing personal letters of recommendation.

Transcript Release Form for CURRENT students
All students must have this form filled out before we send out their official high school transcript.
Whenever possible, PCHS will send the student transcript electronically to the receiving college or university.

(Note: This release form is for current students only - PCHS Alumni should complete and send this form to the Registrar's office.)

Final Transcript Release Form
This form is for the Class of 2012 ONLY. Seniors need to fill out this form with the name and address of the college/university that they will be attending after graduation. It should also be indicated if transcripts should be sent to the NCAA or NAIA and/or if the student will be attending summer school at a college or university in July or August 2012. This form is due to the Guidance Office by Friday, May 4.
