Registration for New Students

Registration forms for the Class of 2016 are to be completed online.

If you were not able to attend Scheduling Night in February 2012, it is imperative that you contact Mrs. Rachel Ellingson by email or at 815-717-3160 prior to beginning the online registration process. Mrs. Ellingson will send you the link for the online registration site.

Click here for our Registration Tips. We strongly recommend that you print and follow the instructions on this document. It will provide answers to many of your questions.  The registration form will take approximately 30 minutes to complete.

Have on hand before you begin:

1. Addresses, phone numbers and email addresses for all parents/guardians of your student. We will need information for EACH parent who should receive correspondence from Providence Catholic.


2. Name and district number of your Public High School.


3. Credit card information if you choose to pay online. Otherwise, the $300 deposit can be mailed in advance or brought to Scheduling Night on Feb. 27.


4. Birth Certificate (can be scanned and uploaded, mailed in, or brought to Scheduling Night)
