The college counselors meet with the entire senior class at the beginning of the school year. The following forms are used for the college application and admission process.
This handout gives detailed information on the application process at PCHS. These rules are to be followed by all students.
Application Tracking Slip
This form MUST accompany ALL transcript requests whether you apply online or with a paper application.
College Planning Packet
We ask that the students give us information about themselves. Counselors use this packet to help with writing personal letters of recommendation. The information is also used for scholarship purposes.
Info Sheet for Teacher’s Letter of Recommendation
This form is filled out by the student for each teacher/coach who will be writing a letter of recommendation. After completed, this form is returned to Mr. Murphy or Mr. Palmasani for review. After their review, it is returned to the student who will then give it to the teacher/coach who is writing the letter of recommendation.
Parent Brag Sheet
This gives the parents a chance to brag about their student. We will use this information to help us with writing personal letters of recommendation.
College Day Verification
Students take this form on their official college visit which is signed by a representative of the college. At least one week prior to the visit, the student MUST bring a note to the Guidance Office signed by a parent stating that the student will be taking their official college day.
Students are allowed one official college day which MUST be taken by May 1, 2016. This is an excused absence and students are responsible to making up any missed homework, test, quizzes, etc.
Transcript Release Form
All students must have this form filled out before we send out their official high school transcript as part of the college application process. Whenever possible, PCHS will send the student transcript electronically to the receiving college or university.
(Note: This release form is for current students only – PCHS Alumni should complete and send this form to the Registrar’s office.)
Final Transcript Release Form
This form is for the Class of 2017 ONLY. Seniors need to provide the name and city of the college/university that they will be attending after graduation. They should also indicate if transcripts should be sent to the NCAA or NAIA and if the student will be attending summer school at a college/university in July or August 2017. The form must be signed by a parent or the student if he/she is at least 18 years old. This form is due to the Counseling Office by Friday, May 5, 2017.
