Transcript Request for Alumni
Please Note: Transcript requests received between April 17 and April 21 will be processed on Tuesday, April 22 upon our return from Easter break.
These procedures are for Alumni only. Current students should contact Mrs. Brassil in the Counseling office during school hours.
PCHS must receive an online request or a completed request form by mail to send all transcripts. FORM IS AVAILABLE AT BOTTOM OF THE PAGE.
- Allow two regularly scheduled school days to process transcripts
- $4.00 processing fee per online transcript request
- Transcripts requested to be sent to you or a business (instead of educational institution) will be sent by US mail only.

Print a transcript request form(s) and mail with payment to:

*Please include check made payable to PCHS in the amount of $4.00 per transcript request.
Online Request/Payment
By completing the transcript request form below, you are authorizing PCHS to send your official high school transcripts to the institution(s) you specify.
Once you select the “Continue to Paypal” button at the bottom of the form, you will be redirected to the safe and secure Paypal website, which will process your request. Don’t have Paypal? You may use any major credit card through Paypal services.
Once the transaction is complete, your transcript request will be sent electronically to our registrar for processing.
