- PCHS must receive an online request or a completed request form to send all transcripts
- Allow two regularly scheduled school days to process transcripts
- $4.00 processing fee per online transcript request <or>
- $3.00 processing fee per mailed transcript or requests submitted in person

*Please include check made payable to PCHS in the amount of $3.00 per transcript request.
In Person Request
Transcript request forms may also be completed in the school’s Main Office between 7:15 AM – 2:45 PM on regularly scheduled school days. (Check the website for summer hours.) Pease call ahead of time to confirm availability at (815) 485-2136. The Main Office staff will accept the $3.00 fee per transcript in the form of cash or check made payable to PCHS.
By completing the transcript request form below, you are authorizing PCHS to mail your official high school transcripts to the institution(s) you specify.
Once you select the “Continue to Paypal” button at the bottom of the form, you will be redirected to the safe and secure Paypal website, which will process your request.
Don’t have Paypal? No problem! You may use any major credit card through Paypal services.
Once the transaction is complete, your transcript request will be sent electronically to our registrar for processing.
